Sometimes, you just need a little help to keep up with daily activities. Other times, you may need more extended care. In either case, we believe it’s your right to stay in your home and community for as long as you choose—and we’re here to provide support to the best of our ability to make that possible.
Mystic Valley Elder Services home care program helps individuals age 60+ and adults living with disabilities who need help to maintain their independence and wellbeing. Many services are provided at low or no cost. Other services are available on a sliding scale to fit your income. One of our expert care managers will help you develop a plan suited to you.

How It Works
When you enroll in our home care program, a compassionate, highly professional care manager will visit you where you live. Together, you’ll design a personalized care plan that addresses your goals and needs. Your care manager will then get your services started and will be there to check in and make adjustments for as long as you need assistance.
You are eligible if:
you are 60 years of age or over or an adult living with Alzheimer’s disease or other dementia related illnesses;
you have needs for personal care assistance or with other needs considered as activities of daily living
Frail Elder Waiver
Home- and Community-Based Services Waivers (HCBS) make community supports available to MassHealth-eligible people who would otherwise require care in a facility. The Frail Elder Waiver (FEW) is an HCBS program through MassHealth. FEW assists individuals whose needs can be met with supports that range from basic to intensive levels. Massachusetts residents aged 60 and older are eligible. The Massachusetts Executive Office of Elder Affairs (EOEA) manages the day-to-day operation of the FEW program.
