Highlights of 2024 from Around Mystic Valley Elder Services

Mystic Valley Elder Services (MVES) is thankful to the many partners and individuals, including our staff, volunteers, donors, and community-based organizations, that help our organization fulfill our mission to support older adults and people with disabilities age with dignity and respect throughout the communities we serve.

As 2024 comes to an end, we wanted to send a quick note to highlight a few of the ways your contributions and efforts have helped our organization to make a difference in the lives of those in need this year:

Enrollment in our State Home Care and Managed Care Programs increased by 5% in 2024. These programs enable 8,889 consumers to receive essential home and community-based services to remain safely in their own homes.

Our weekly on-site Wellness Nurse services were expanded to five Supportive Housing locations, allowing MVES to reach 755 older adults with vital sign checks and health education events.

Through a partnership with Health Care for All and MassHealth, MVES helped individuals navigate Medicaid Redetermination by holding 86 outreach events reaching over 2,250 people.

Attendance at the community congregate meal sites we offer in collaboration with local Councils on Aging and Elder Supportive Housing sites increased by 29%.

Our Hospital to Home Partnerships with Tufts MelroseWakefield Hospital, Cambridge Health Alliance-Everett, and Winchester Hospital aided 480 people transitioning from hospital stays back home through individualized community care planning.

With funding through a Respite Innovation grant, our Family Caregiver Support Program supported over 150 family caregivers and their loved ones through new co-located programming including art classes, cooking demonstrations, and a unique equine therapy series in partnership with Ironstone Farm.

To prepare our team to best meet the needs of the communities we serve, MVES provided an extensive Diversity, Equity, Inclusion, and Accessibility (DEIA) Initiative for our staff, which reached over 350 employees with 73 DEIA training sessions.

MVES also received funding from the Commonwealth to expand our Behavioral Health Outreach for Aging Populations (BHOAP) Program which provided 286 individuals with connections to behavioral health services and supports.

In 2025, we will celebrate our Golden Anniversary, marking 50 years of service since our founding in 1975. Now, more than ever, you remain essential to our continued growth and success.
